Practice Policies

We ask for at least 24 hours' notice if you need to reschedule your appointment. Cancellations made with less than 24 hours' notice are considered late cancels.

Arriving 10 minutes or later to your appointment will be marked as a no-show, and your appointment will need to be rescheduled.

There is an $80 fee for late cancellations and no-shows, which is not covered by insurance.

Please note that three late cancellations or no-shows may lead to discharge from the practice.

Payment, including co-pays, are due at the time of the appointment. If you have barriers to this, please let your therapist know & alternate arrangements may be made.

You are required to complete initial paperwork at least 24 hours prior to the first session, which includes:

• Client contacts form
• Client history form
• Client information form
• Client insurance form
• Consent for services
• Telehealth informed consent
• Release of information
• Notice of privacy practices
• Copy of front & back of insurance card
• Copy of government-issued identification card

You may also be required to complete this paperwork on an annual basis.
